Health and Wellbeing Coaches
Do you want to live a healthier lifestyle but you aren’t sure where to start?
By making small changes to your daily habits such as eating well, staying active, getting enough rest, and practicing self-care, you can improve your health and quality of life. Our Health and Wellbeing Coaches can support, educate, and motivate you to improve the four pillars of health: diet, activity levels, sleep, and stress.
We want to help those who want to make a change. Our Health and Wellbeing Coaches are there to help you achieve your personal goals by helping you understand your goals, overcoming barriers, and making positive changes.
After understanding your concerns, we can connect you to services like Activity and Referral, Social Prescribing Link Workers, Talking Therapies, Sleep Station, and other activities to provide additional support for your health.
Length of Intervention
Our service is around 12 weeks on average.
Eligibility Criteria
If you are 18 years+ and you are looking to make a positive change, our Health and Wellbeing Coaches are ready to help you to achieve your goals and lead a heathier life.
You don’t need to have a specific condition or be facing a specific challenge, as long as you want to make a change, we want to help you. We know that change can be challenging, and we have to be ready to make it, which is why we won’t limit people to one go. If you try and it’s not the right time, you can always come back to us when you’re ready.
Unfortunately, we can’t currently support people with carers or those in mental health crisis.
How do I refer myself?
Our service is available at selected GP practices. If your GP practice is listed, please speak to reception or with a Clinician about our service and someone will be in contact you for a discussion.
Frequently Asked Questions
We are a coaching service, not a counselling service. While we help people unpick some of the behaviors that are holding them back, if we feel we’re not qualified to help you, we will refer you to a more appropriate service. We won’t leave you without any help or advice.
One-to-one sessions over the phone or video call with some limited face-to-face sessions offered.
The first sessions are between 45 minutes to 1 hour; anything after that is between 30 minutes to 45 minutes.
If at the end of 12 weeks, you feel as though you need additional time or support, we offer the Wellbeing Launchpad for additional services and resources to support your overall health.
Case Study
Story
A client in her 70’s, lives with her husband and used to be a swimming teacher as she was passionate about teaching people to swim. However, she was diagnosed with arthritis and a back injury, leading to chronic pain and limited physical activity.
Goals
The client felt stuck, demotivated, and was experiencing low self-esteem. She had aims to get more active and start swimming again, however, she felt self-conscious about her appearance in a swimsuit. She was determined to lose weight to feel good about herself again.
Solutions
The Health and Wellbeing Coach sat with the client to set goals to motivate her. She was referred to the SPRING Chronic Pain Pathway so she can work towards being more active and start swimming again. They set achievable goals such as buying a swimsuit she felt comfortable in and walking around the park or even just the end of the road to start off with. The client was also advised to start a food diary, so she was able to monitor her eating habits and stay on track.
Outcomes
After speaking with the Health and Wellbeing Coach, the client built her confidence and started to swim again. She feels great and focuses less on her weight and more about how she feels. She is slowly building her stamina and walking longer alongside her walking buddy, who she has recently met. She finally feels empowered and stronger to do the things she thought she couldn’t do.
