If you would like to book an NHS Health Check, please contact your registered GP practice.

During normal GP hours: please contact your registered GP practice and they will cancel/rearrange your appointment on your behalf.

Outside of normal hours: please call the GP Enhanced Access hotline to cancel directly (this telephone number will be provided when your appointment is booked.)
