Care Coordinators play an important role in making sure you receive organised care. We help connect you to services by working with various healthcare professionals such as GPs, Pharmacists, Nurses, Social Prescribing Link Workers, and Health and Wellbeing Coaches.
Through our dedication and teamwork, Care Coordinators contribute to improving your overall health and wellbeing concerns.
What is the role of a Care Coordinator?
Care Coordinators can support you by ensuring you are getting the support you need to live healthy, fulfilled lives by linking you to other services such as Social Prescribing Link Workers, Health and Wellbeing Coaches and other professionals where appropriate.

- If you have long-term health conditions
- If you require a follow up clinical conversation
- If you need to be linked to additional services such as Social Prescribing Link Workers, Health and Wellbeing Coaches, or other appropriate professionals
- If you require support in Care/Nursing Homes
How can I be referred to a specific service?
If you feel a specific service may help you or someone you care for, please speak to a GP practice member, and a Care Coordinator will be in touch.
Care Coordinators work Monday to Friday, during usual practice opening hours, generally between 9:00 to 5:00.
