We are excited to be recruiting for an enthusiastic Occupational Therapist to join our Collaborative Care Team (CCT), working full time on a 12-month FTC.
The primary focus of GPA’s Collaborative Care Team is to deliver out-of-hospital care, supporting patients at their place of residence to foster and maintain independent living.
As a CCT Occupational Therapist, you’ll connect service users, primary care, and support organisations, tackling health inequalities in long-term conditions. Your role includes nurturing independence, facilitating reablement, and promoting effective partnership-working
